Vision Insurance
Many companies now provide vision coverage for their employees. This coverage is separate from your regular health insurance and is often administered by a different provider. If you are unsure whether you have vision coverage, check with your human resource department. Policies are not only different from one provider to another, but the same provider may offer many different vision policies. Although we will try to assist you in any way we can, it is your responsibility to know the benefits and requirements your company offers.
Although we will file your insurance for you, that is not a guarantee that you are eligible for benefits. We are panel providers for several large vision care plans that allow us to check eligibility but there are many smaller plans that we are unable to check coverage. Plans that we are Panel Doctors for include:
- Medicare
- Vision Service Plan (VSP)
- United Health Care (Coordinated Vision Care - CVC)
- Eyemed / Eye Care Plan of America
- Vision Plus
- Cole Vision
- Avesis
- Davis Vision (only certain plans)
- Spectera
Insurance Filing Policy
We are committed to providing you with the best possible care. In order to achieve this, we need your assistance and your understanding of our insurance filing policy.
We must emphasize that as your vision care provider our relationship is with you, not your insurance company. While the filing of insurance claims is a courtesy that we extend to you, all charges are ultimately your responsibility from the date the initial services are rendered. We will gladly file insurance claims for you to your insurance company, however, in the event that the claim goes unanswered or is denied, the balance due will be billed to you. Also, please keep in mind that we accept many different insurance plans. Knowledge of the terms and eligibility requirements of your particular plan is your responsibility.
Re-filing of claims due to incomplete or inaccurate information provided to us may incur a $10.00 re-filing fee. Charges not covered by insurance should be rendered at time of service unless other arrangements are made in advance. Thank you.
