Then choose a date, fill in the information indicated, and click on "send". Your request is emailed to our office. We will call you back to confirm the appointment. If that particular time is not available we will present you with the options closest to your request. For new patients, print and fill out the "Welcome Form" and bring it with you to your 1st appointment.
